PANEL DISCUSSION IV SURGERY FOR ARTERIOSCLEROSIS OBLITERANS: COMMENTS
Based on this Panel discussion, the current status of the surgical treatment, including indications, techniques and results, was reviewed. Because arteriosclerosis obliterans is the partial phenomenon of the generalized atherosclerosis, it is essential to observe the patients, not only from the surgical point of view, but also through the general examination, to make the final plan of the treatment.
On this occasion, I would like to introduce two recent subjects on this problem. The first one is abdominal angina, which has been until now rarely observed but will increase in near future also in Japan. It is commonly agreed that these patients should be treated surgically, once the diagnosis established, to correct the symptoms and to prevent the eventual progression to intestinal infarction. The successful case reports with arterial reconstruction has recently increased in number.
The second is surgery for hyperlipidemia. It is well known that hyperlipidemia is one of riskfactors, which accerelate the progression of atherosclerosis. Although the surgical treatment aimed up to date at the lesions caused by atherosclerosis, it will be mandatory to supress its progression in the future. In this meaning, intestinal bypass and portacaval shunt for hyperlipidemia are noteworthy.
