ARTERIOSCLEROSIS OBLITERANS
-OPERATIVE INDICATION FOR AND METHOD OF AORTO-ILIAC OCCLUSION-

1st Department of Surgery, Gifu University, School of Medicine

Mitsuo Hirose

Experiences with 152 cases of arteriosclerosis obliterans was reported. Involvement of branch of aortic arch is rare, counting only five of them. Average age of patients, localization of occlusive lesions and frequency of reconstructive surgery in occlusive cases of lower extremities are same as in the case of Western people. Incidence of complications is low in this series compared to reported series in English literatures, especially myocardial infarction and diabetes mellitus, although hypertension is common and abnormal electrocardiographic findings are prevalent.
Thromboendarterectomy was a choice of surgical method for aorto-iliac occlusion, while vein bypass graft was preferred for cases with femoro-popliteal occlusion. No extra-anatomic bypass was done in this series. Cumurative patency rate in aorto-iliac reconstruction was 82.5 per cent during seven years.
Limb salvage was good except cases with acute occlusion, resulting in 9 cases of major amputation in this series. Five year survival rate after surgery is estimated about 72 per cent.
