ARTERIOSCLEROTIC RENOVASCULAR HYPERTENSION
―SURGICAL TREATMENT AND PROGNOSIS―
32 cases of renovascular hypertension were operated upon at Y.C.U.H.. 11 cases of these were caused by arteriosclerosis. The age ranged from 37 to 69 years old, average 52 years old. Male were in 10 cases and female was single. Operation was decided by plasma renin activity, split renal function, renogram, intravenous pyelogram and angiography. Thromboendarterectomy and autosaphenous vein patch graft were made in 7 cases and 4 cases were nephrectomized. Postoperative blood pressure fell to normal range in 67% of reconstructive cases, whereas, improvement of blood pressure were obtained in all 4 cases of nephrectomy. Blood pressure gradually elevated in longterm period in some cases.
ST depression of ECG and cerebrovascular accident indicated poor prognosis. Our cases with these findings in 4 of 7 cases were died during 7 years 9 months postoperatively. Causes of death were cardiac in three cases, cerebrovascular disease in one case. Hypertension was not concerned with causes of death.
General status of atherosclerosis should be examined carefully before operation.
