SURGICAL APPROACH WITH THORACOTOMY FOR CARCINOMA OF THE CARDIA

1. Our operative approach for excision of tumor and dissection of regional lymphnodes in the treatment of carcinoma of the cardia through short upper midline incision combined with left thoracotomy and division of the diaphragm was demonstrated.
2. Fifteen patients were undergone this approach and showed excellent postoperative courses without any operative motality.
3. The procedure had clinical advantages of lower pain at wounds and of less disturbances on pulmonary gas exchange in postoperative period since it had shorter abdominal incision than usual combined thoracoabdorninal approach. Postoperative pulmonary complication was found in only one case (7%).
4. The approach resulted so much wider operative field that radical operation with wider dissection of lymphnodes could be easily performed without residual cancer in the stump.
5. This operative method should be applied as an efficient approach in the treatment of cancer of the cardia.
