VARIATION IN THE PORTAL CIRCULATION AFTER HEPATECTOMY IN DOGS (Experimental Study)
We divided experimental dogs into four goups. A group are 0-30% hepatectomized dogs, 31-50% are B group, 51-80% are C and 81-100% are "D". At the each groups, we researched into the portal tension, the portal circulation time (dys excretory curve), and the portal blood flow periodically after partial hepatectomy.
The results were as foIIows:
"A" (0-30%)
Portal tension : 10 mmH
2O rised
Portal circulation time : normal Compensating
Portal blood flow : 14% decreased Stadium I
"B" (31-50%)
Portal tension : 21 mmH
2O rised
Portal circulation time : nearly normal Compensating
Portal blood flow : 20% decreased Stadium II
"C" (51-80%)
Portal tension : 40 mmH
2O rised Relative
Portal circulation time : varied decompensating
Portal blood flow : 25% decreases Stadium
"D" (81-100%)
Portal tension : 112 mmH
2O rised
Portal circulation time : varied Absolute
markedly, such as the dys passed decompensating
through the canal transiently Stadium
Portal blood flow : 70% decreased
From the results mentioned before :
We informed of that the portal circulation broke up suddenly about 90% hepatectomized ones. Contrary, as far as 80% hepatectomy, portal circulation did not varied markedly.
We decided that the maximum of partial hepatectomy was 70% resection. We researched for how to be compensated the restricted portal vascular condition by the partial hepatectomy.
For the purpose of it, by means of rapid serial angiography, the vascular arrangement in the liver has been studied.
The characteristic manifestations were as follows :
(1) Portal blood does not always perfuse the entire liver, the circulation is maintained through the hilar region (vascular hilum) of the organ. So-called cutting feature of portal branch (stem-branch and peripheral branch). Cutting feature of peripheral branch appears after within 50% hepatectomy. Cutting feature of stem-branch appears after over 50% hepatectomy. Cutting feature disappears within 2-3 weeks after partial hepatectomy.
(2) The flected and abnormal remained shadows of the contrast medium appear around the cutting feature of the portal branch, the more hepatectomy be great, the more they be distinguishable.
(3) Rapid appearanc of the inferior caval vein shadow
(4) Refluence feature of the extra hepatic portal branch
(5) Others
