MANAGEMENT OF DISSECTING ANEURYSM OF THE AORTA USING DOUBLE RINGED GRAFT
We have treated 24 patients of dissecting aneurysms of the aorta during past 3 years. In these patients 10 cases were performed surgical operation. In the cases before 1978, aneurysm was excised and replaced with a graft. In these experiences hemorrhage from the suture line was sometimes uncontrollable and fatal. To avoid these complication we used a new technique utilizing a double ringed prosthesis. The device used is a segment of a woven dacron graft with two stainless steel rings at both end. The distance between two rings is made 10ー12mm. Operative technique is as follows. The prosthesis lies inside the true lumen of the dissected aorta, tape is ligated and sutured arownd the aorta just fized between two rings. Suture of the aortic layers overlying the inserted graft completes the procedure. Using this new technique 5 cases involved acute stadium of dissection of the aorta were performed surgical operation. Among these cases, 3 cases were successfully and 2 cases died postoperatively. From these experience, we emphasizet hat this new technique is simple and can be used safely in the repair of the dissecting aneurysm in any part of the aorta, whether it is acute or chronic.
